ILA License Plate Program

Do you want to support Illinois Lyme Association in furthering our mission? ILA is announcing our license plate and sticker decal program! We passed a statute that designates a permanent specialty license plate and sticker decal with the Secretary of State’s office, but we need your help! They will only begin production of the decals if we reach 2,000 requests by January 2026.

There are just 3 simple steps to request your license plate decal:

• Fill out the form below.

• Submit credit card payment in the form.

• Signature is REQUIRED.

Help us reach our goal of 2,000 requests by January 2026! If we reach this goal, the plate and specialty decal sticker becomes an ongoing program, giving ILA a permanent revenue stream.

But even if we don’t reach this amount by the deadline, all the money from the fees will still directly support ILA!
